Welcome to Norfolk's Your Guide to Care and Support for Adults, a resource intended to guide you throughout adult care and support in the county in 2025/26. It aims to provide information to help you, and anyone you know, stay independent, safe and well for as long as possible, as well as details of services and organisations to assist along the way. Health and social care work together to both support people in, or at risk of, crisis and hospital admission and also give support for those returning from a hospital stay. Other organisations and services also offer support to people in temporary difficulty. We hope this guide helps you to consider what your choices are and find the information and services you need to make the choices that are right for you.
